It deserves a closer look. Delayed notification about injuries and conflicting explanations from staff members are both among the warning signs that something may warrant closer attention, alongside unexplained injuries, repeated falls, and sudden changes in behavior.

A delay does not by itself establish neglect, and not every injury means abuse or neglect occurred. However, preventable harm should not be dismissed simply because someone is older or needs long-term care.
