Ask what happened, when staff discovered the problem, and how they responded. It also helps to ask what services the facility was supposed to provide, what assistance your loved one required, whether those needs were documented, and what the care plan required.

Pay attention when explanations change or important questions remain unanswered. Writing down dates, injuries, conversations, and staff explanations as you go makes it easier to reconstruct events later, and photographs may help document visible conditions when appropriate.
